VETERANS AFFAIRS

Veterans are welcome at Face Value Dental and we are proud to participate in the delivery of dental treatment to Veterans and their families. If you are a gold card holder then you have full treatment entitlement in accordance with the schedules as issued each year by the Department of Veterans Affairs.

SENIORS CARD

Seniors Card holders are entitled to a 7% discount of all services. All Queensland residents aged 60 or over are eligible for either a Seniors Card or Seniors Business Discount Card.

As an MBF member Core Provider we charge MBF members in accordance with the MBF schedule. You will receive the maximum entitlement in accordance with your level of insurance.

There are limits on the number of times per year you can claim a benefit for these services. Preventive dental services covered by MBF MemberCare include:
•  Initial and periodic oral examinations
•  Intraoral periapical or bitewing radiographs
•  Removal of plaque
•  Removal of calculus (first and subsequent visit)
•  Topical application of fluoride
•  Oral hygiene instruction
•  Provision of a single arch mouthguard (indirect) or bimaxillary mouthguard
•  Fissure sealings.
•  Simple restorations

All benefits paid are subject to normal waiting periods and the MBF Dental Guidelines.

CareCredit offers a range of Interest Free* and Extended Payment* options for treatments from $500 to over $25,000. Please note that these are offered at the discretion of the practice.

Interest Free*
•  12 months Interest Free available
•  No interest if the balance is paid within the specified interest free period
•  Low minimum monthly payments (3% of the total closing account balance or $40, whichever is greater). Paying the minimum monthly repayment will not pay out the loan before the end of the interest free period.
•  Annual percentage rate of 18.9% at the end of the interest free period
•  Ability to make additional repayments or payments greater than the requested 3% minimum during the Interest Free promotional period without any penalties

Extended Payment*
•  Choice of 12,24 and 36 month Extended Payment Plans

Fees & Charges
•  A once only establishment fee of $25 applies
•  If you do not have an active account balance on your card, then you will not be charged any other fees at all
•  When you do actively use your card and have an active balance, fees and charges will apply, including a monthly account service fee of $2.95
•  When you apply for CareCredit, you will be told about all the applicable fees and charges before you progress with your application

HICAPS

Hicaps (medical benefits insurance processing) can be done on the spot. All your insurance benefits are paid directly to us and you are only responsible for the gap payment. Make sure you bring your health fund card when you visit us.

HCF

You will receive dental services in accordance with the MCF prefered provider fee schedule. The rebate you receive from MCF will be in accordance with the terms of your insurance policy with them.

CREDIT CARD / EFTPOS / CHEQUE / CASH

Credit cards (AMEX, Visa and MasterCard), EFTPOS, Personal Cheque and cash are all accepted methods of payment.
